Summer Camp: Required Downloadable Forms |

In an effort to make it easier for families to complete registration, we provide each of the forms required for full Summer Camp or Wilderness Camp registration below.
Simply download each form and fill it out, then email them back to us. If you prefer to have these forms emailed or mailed to you, please let us know and we will be happy to send them to you.
Each form is important; please read each one carefully while filling it out. All forms should be returned to us no later than 30 days prior to the start of your camper's session.
Each camper is required to have a completed health history and health exam (physical) prior to camp attendance. A new health exam is required every two years and must be filled out by a licensed Physician.
Note: You can download each form individually below as .pdf's, or use the appropriate .zip links to download all the forms at once in a single .zip archive (if you are familiar with .zip and know how to extract the package).
